1. Scope
Revivex Healthcare, Inc., doing business as Pacific Pain & Wellness Group ("PPWG," "we," "us," or "our"), provides this Online Privacy Policy for the public websites, public subdomains, and online services that link to it (the "Online Services"). These include pacificpaingroup.com, ketamine.pacificpaingroup.com, wellness.pacificpaingroup.com, and intake.pacificpaingroup.com, subject to the different privacy controls appropriate to each service. The Online Services are intended for people in the United States. Information submitted through the Online Services is transferred to and processed in the United States.
This Policy does not replace PPWG's Medical Information Notice of Privacy Practices, which governs protected health information where HIPAA applies. It also does not govern independent third-party services such as patient portals, telehealth providers, payment processors, questionnaires, document-upload services, or social-media platforms; those services provide their own notices.
2. Information We Collect
Depending on the service and how you use it, we may collect:
- identifiers and contact information, such as name, email address, postal address, telephone number, and account identifier;
- information you submit in a public contact or secure intake form;
- health, treatment, insurance, payment, identification, or appointment information when you use a designated secure service;
- device and network information, such as IP address, browser type, operating system, language, approximate location derived from IP address, and log data;
- online activity information, such as pages viewed, links selected, referring source, date and time of a visit, and consent choices;
- communications and records of requests you send to us; and
- information received from providers that perform services for PPWG or from other sources where permitted by law.
Do not provide medical information through a public automated assistant, general email, or a public social-media page. Use PPWG's designated secure channels for medical, insurance, identification, or treatment information.
3. How We Use Information
We may use information to provide requested services, communicate with you, respond to requests, operate and secure the Online Services, schedule or coordinate care, process payment where applicable, meet legal obligations, protect health and safety, prevent fraud and misuse, improve services, and maintain records.
We use nonessential analytics and advertising technology only in accordance with the Cookie Policy and applicable privacy choices. We do not permit patient form values, medical records, diagnoses, symptoms, medication information, insurance information, or secure-workflow information to be sent to general advertising or analytics services.
4. How We Share Information
We may share information with PPWG workforce members, affiliated entities, service providers, and Business Associates as needed for the purposes described above and as permitted by law. We may also disclose information in connection with a legal requirement, health or safety emergency, corporate transaction, or to protect rights and security.
Where a service provider creates, receives, maintains, or transmits protected health information for PPWG, PPWG will evaluate whether a Business Associate Agreement is required. We do not sell protected health information.
5. Communications and Preferences
Where permitted by law, PPWG may communicate by phone, text, email, mail, portal, or other method you provide. You may ask about communication preferences or opt out of promotional communications using the method provided in the message. Opting out does not apply to communications necessary for treatment, payment, care coordination, safety, account administration, or legal obligations.
PPWG will not share mobile opt-in data or text-message consent with third parties or affiliates for their marketing or promotional purposes. Messaging terms, consent language, and opt-out instructions must be provided with each applicable program.
6. Cookies, Analytics, Advertising, and Global Privacy Control
PPWG uses cookies and similar technologies as described in the Cookie Policy. Strictly necessary technologies support security, operation, and consent preferences. Analytics, functional, and advertising technologies are controlled through PPWG's consent-management process where applicable.
PPWG recognizes supported Global Privacy Control signals as an opt-out of sale or sharing for cross-context behavioral advertising where required by applicable law. A cookie choice is not a HIPAA authorization. PPWG restricts general analytics, advertising, session replay, and public chat from secure intake, upload, authenticated, payment, and confirmation workflows.
7. California Privacy Rights
This section applies where the CCPA/CPRA applies to PPWG and the relevant information. California residents may have rights to know, access, delete, correct, opt out of sale or sharing, limit certain uses of sensitive personal information, and not be discriminated against for exercising privacy rights. PPWG will verify requests as required by law and will explain any lawful limitation or exemption.
Where required, a notice at collection will identify the categories of personal information collected and the purposes for which the information will be used. It will also provide a link to this Policy and any applicable opt-out or limitation mechanism.

PPWG will provide the verification instructions and any additional request method required by law for the applicable request. An authorized agent may submit a request where permitted by law, subject to verification of the agent's authority and the requester's identity.
8. Retention and Security
PPWG retains information for the period reasonably necessary for the disclosed purpose, applicable legal requirements, records-retention obligations, dispute resolution, and security. Retention periods for patient, intake, consent, analytics, chat, and marketing data must be documented in PPWG's internal retention schedule.
PPWG uses administrative, technical, and physical safeguards appropriate to the nature of the information. No online system can be guaranteed completely secure; do not send urgent or confidential health information through a channel PPWG has not designated for that purpose.
9. Children
PPWG does not knowingly collect personal information from children under 13 through public Online Services without required parental consent. A parent or guardian who believes a child provided information through a public service may contact PPWG using the information below.
10. Social Media
PPWG social-media pages are public environments controlled in part by the platform operator. Do not post medical, insurance, financial, account, or identifying information on public pages. PPWG may remove or report content that is unlawful, unsafe, misleading, infringing, abusive, or inconsistent with page rules, but it cannot guarantee removal or control third-party posts.
11. Social Security Number Protection
PPWG protects Social Security numbers it receives or collects through physical, technical, and administrative safeguards appropriate to the information. Access is limited to authorized persons with a lawful business need, and unlawful disclosure is prohibited.
12. Changes and Contact
PPWG may update this Policy when its practices, services, vendors, or legal obligations change. The current version will include an effective or last-updated date.
